Emphysema

  1. Definition Emphysema is a chronic lung disease characterized by progressive damage to the alveoli (air sacs) in the lungs, leading to shortness of breath and difficulty breathing. [Damage to air sacs in lungs]

  2. Causes / Etiology Emphysema is caused by long-term exposure to lung irritants such as cigarette smoke, air pollution, and chemicals. It can also be inherited in some cases. [Long-term exposure to lung irritants]

  3. Types or Classification

    • Centriacinar Emphysema: Characterized by damage to the central airways, often due to cigarette smoke.
    • Panacinar Emphysema: Affects the entire lung tissue, often due to genetic factors.
    • Distal Acinar Emphysema: Affects the smaller airways, often due to long-term exposure to lung irritants.
    • Irreversible Emphysema: A type of emphysema that cannot be reversed with treatment.
  4. Pathology β†’ Step 1: Long-term exposure to lung irritants damages the alveoli, leading to inflammation and scarring. β†’ Step 2: The damage causes the alveoli to lose their elasticity and ability to expand and contract. β†’ Step 3: The damaged alveoli become filled with fibrotic tissue, leading to decreased lung function. β†’ Step 4: The lungs become stiff and less capable of exchanging oxygen and carbon dioxide.

  5. Clinical Features

    • General Features: Shortness of breath, wheezing, and coughing.
    • Specific Features: Bluish discoloration of the skin (cyanosis), clubbing of the fingers, and weight loss.
  6. Complications

    • Acute Complications: Respiratory failure, pneumonia, and heart failure.
    • Chronic Complications: Cor pulmonale, pulmonary hypertension, and malnutrition.
  7. Diagnosis / Investigations

    • Routine Tests: Chest X-ray, spirometry, and blood tests.

  8. Management

    • Dietary Advice: Eating a balanced diet, avoiding foods that trigger symptoms, and staying hydrated.
