Congestive Cardiac Failure

Definition Congestive Cardiac Failure (CCF) is a condition where the heart is unable to pump blood efficiently, leading to a buildup of fluid in the body's tissues and organs. [Heart failure means the heart cannot function properly.]

Causes / Etiology CCF can be caused by various factors, including:

  • High blood pressure (hypertension) [high pressure in the blood vessels]
  • Heart attack or coronary artery disease [blockage of the heart's blood vessels]
  • Heart valve problems [abnormal heart valves]
  • Cardiomyopathy [disease of the heart muscle]
  • Heart rhythm disorders [abnormal heartbeats]
  • Medications or toxins that affect the heart [certain medicines or poisons]

Types or Classification There are several types of CCF, including:

  • Right-sided heart failure: involves the right side of the heart and can cause fluid buildup in the legs, abdomen, and liver.
  • Left-sided heart failure: involves the left side of the heart and can cause fluid buildup in the lungs.
  • Biventricular heart failure: involves both sides of the heart and can cause fluid buildup in multiple areas of the body.
  • Systolic heart failure: involves the heart's ability to contract and pump blood.
  • Diastolic heart failure: involves the heart's ability to relax and fill with blood.

Pathology The pathophysiology of CCF can be explained in the following steps:

Step 1 โ†’ The heart muscle becomes damaged or weakened due to various causes, such as high blood pressure or heart attack.

Step 2 โ†’ The heart's ability to pump blood is impaired, leading to a buildup of fluid in the body's tissues and organs.

Step 3 โ†’ The fluid buildup can cause swelling, shortness of breath, and other symptoms.

Step 4 โ†’ The heart's ability to relax and fill with blood is also impaired, leading to further fluid buildup and symptoms.

Clinical Features The clinical features of CCF can include:

General features:

  • Shortness of breath (dyspnea) [difficulty breathing]
  • Fatigue
  • Swelling in the legs, ankles, and feet
  • Coughing or wheezing

Specific features:

  • Right-sided heart failure: swelling in the legs, abdomen, and liver
  • Left-sided heart failure: swelling in the lungs
  • Biventricular heart failure: swelling in multiple areas of the body

Complications The complications of CCF can include:

Acute complications:

  • Pulmonary edema [fluid buildup in the lungs]
  • Cardiac arrest [heart stops beating]
  • Sepsis [blood infection]

Chronic complications:

  • Heart failure progression
  • Kidney failure
  • Liver failure
  • Malnutrition

Diagnosis / Investigations The diagnosis of CCF can be made using:

Routine tests:

  • Electrocardiogram (ECG) [heart rhythm test]
  • Chest X-ray [image of the lungs and heart]
  • Blood tests [to check for fluid buildup and other complications]

Special tests:

  • Echocardiogram [image of the heart]
  • Cardiac MRI [image of the heart]
  • Cardiac catheterization [to check for blockages in the heart's blood vessels]

Management The management of CCF can include:

General management:

  • Rest and relaxation
  • Dietary changes [low-sodium diet]
  • Medications [to reduce fluid buildup and improve heart function]

Modern treatment:

  • Heart transplantation [new heart]
  • Ventricular assist device (VAD) [to support the heart's function]
  • Cardiac resynchronization therapy (CRT) [to improve the heart's rhythm]

Dietary advice:

  • Eat a low-sodium diet
  • Increase fluid intake
