Content

Chronic Non Suppurative Otitis Media

  • Definition: A chronic inflammatory condition of the middle ear characterized by chronic eustachian tube dysfunction, mucosal hypertrophy, and atelectasis of the tympanic membrane.
  • Types:
    • Eustachian catarrh: A condition characterized by chronic eustachian tube obstruction, leading to eustachian tube dysfunction.
    • Serous otitis media: A condition characterized by the presence of fluid in the middle ear cleft without any evidence of infection.
    • Atelectasis of tympanum: A condition characterized by the collapse of the tympanic membrane due to eustachian tube dysfunction.
    • Tympanosclerosis: A condition characterized by the deposition of collagen and calcium in the tympanic membrane.

Eustachian Catarrh

  • Definition: A condition characterized by chronic eustachian tube obstruction.
  • Causes: Allergies, cold, sinusitis, and adenoid hypertrophy.
  • Clinical features: Nasal congestion, eustachian tube dysfunction, and hearing loss.
  • Investigations: Nasal endoscopy, eustachian tube function test.
  • Complications: Serous otitis media, hearing loss, and eustachian tube dysfunction.
  • Management: Treatment of underlying cause, eustachian tube exercise, and hearing aid.

Serous Otitis Media

  • Definition: A condition characterized by the presence of fluid in the middle ear cleft without any evidence of infection.
  • Causes: Eustachian tube dysfunction, allergies, and cold.
  • Clinical features: Hearing loss, eustachian tube dysfunction, and pressure sensation in the ear.
  • Investigations: Otoscopy, tympanometry, and pure tone audiometry.
  • Complications: Hearing loss, eustachian tube dysfunction, and chronic suppurative otitis media.
  • Management: Treatment of underlying cause, eustachian tube exercise, and hearing aid.

Atelectasis of Tympanum

  • Definition: A condition characterized by the collapse of the tympanic membrane due to eustachian tube dysfunction.
  • Causes: Eustachian tube dysfunction, allergies, and cold.
  • Clinical features: Hearing loss, eustachian tube dysfunction, and pressure sensation in the ear.
  • Investigations: Otoscopy, tympanometry, and pure tone audiometry.
  • Complications: Hearing loss, eustachian tube dysfunction, and chronic suppurative otitis media.
  • Management: Treatment of underlying cause, eustachian tube exercise, and hearing aid.

Tympanosclerosis

  • Definition: A condition characterized by the deposition of collagen and calcium in the tympanic membrane.
  • Causes: Chronic otitis media, eustachian tube dysfunction, and trauma.
  • Clinical features: Hearing loss, eustachian tube dysfunction, and pressure sensation in the ear.
  • Investigations: Otoscopy, tympanometry, and pure tone audiometry.
  • Complications: Hearing loss, eustachian tube dysfunction, and chronic suppurative otitis media.
  • Management: Treatment of underlying cause, eustachian tube exercise, and hearing aid.

Blood Supply

  • The tympanic membrane receives blood supply from the arteries of the external auditory canal, including the anterior tympanic artery and the posterior tympanic artery.

Nerve Supply

  • The tympanic membrane receives nerve supply from the auriculotemporal nerve and the nerve of the tympanic membrane.

Clinical Anatomy

  • The tympanic membrane is a thin, semi-transparent membrane that separates the external auditory canal from the middle ear cleft.
  • The eustachian tube opens into the middle ear cleft and is responsible for equalizing the pressure between the external auditory canal and the middle ear cleft.

Homoeopathic Therapeutic Medicines

  • Aconitum napellus: For chronic otitis media with pain and hearing loss.
  • Belladonna: For acute otitis media with fever and pain.
  • Calcarea carbonica: For chronic otitis media with hearing loss and eustachian tube dysfunction.
  • Hepar sulphuris calcareum: For chronic otitis media with pain and discharge.
  • Pulsatilla: For chronic otitis media with hearing loss and eustachian tube dysfunction.
  • Silica: For chronic otitis media with hearing loss and eustachian tube dysfunction.
