Thyroid Cancer
Definition
Thyroid cancer is a type of cancer that originates from the thyroid gland, a butterfly-shaped gland located in the neck.
Types of Thyroid Cancer
- Papillary carcinoma (60-70%): most common type, often spreads to lymph nodes
- Well-differentiated, grows slowly
- Can spread to lymph nodes, distant sites
- Treatment: surgery, radioactive iodine therapy
- Follicular carcinoma (10-15%): also well-differentiated
- Often spreads to lymph nodes, distant sites
- Treatment: surgery, radioactive iodine therapy
- Medullary carcinoma (5-10%): arises from parafollicular cells (C-cells)
- Can be hereditary
- Treatment: surgery, radioactive iodine therapy
- Anaplastic carcinoma (1-2%): poorly differentiated, aggressive
- Often spreads to lymph nodes, distant sites
- Treatment: surgery, chemotherapy, radiation therapy
- Undifferentiated (anaplastic) carcinoma
- Hurthle cell carcinoma
- Thyroid lymphoma
Causes
- Genetic mutations
- Radiation exposure
- Family history
- Previous head or neck radiation
- Radiation therapy for other cancers
- Certain medical conditions (e.g., goiter, thyroiditis)
Clinical Features
- A lump or nodule in the neck
- Swelling or enlargement of the thyroid gland
- Difficulty swallowing or breathing
- Hoarseness or changes in voice
- Coughing or wheezing
- Weight loss or fatigue
- Pain or tenderness in the neck
Investigations
- Physical examination
- Imaging studies: ultrasound, CT, MRI, PET
- Fine-needle aspiration biopsy (FNAB)
- Thyroid function tests (TFTs)
- Thyroid-stimulating hormone (TSH) levels
- Thyroid hormone levels (T3, T4)
- Parathyroid hormone (PTH) levels
Complications
- Spread to lymph nodes or distant sites
- Respiratory distress
- Hoarseness or voice changes
- Swallowing difficulties
- Weight loss or fatigue
- Hypocalcemia (low calcium levels)
- Hypercalcemia (high calcium levels)
Management
- Surgery: thyroidectomy, lobectomy, or subtotal thyroidectomy
- Radioactive iodine therapy
- External beam radiation therapy
- Chemotherapy
- Hormone therapy
- Palliative care for advanced disease
Treatment
- Surgery: primary treatment for most thyroid cancers
- Radioactive iodine therapy: for papillary and follicular carcinoma
- External beam radiation therapy: for anaplastic carcinoma
- Chemotherapy: for advanced disease
- Hormone therapy: for some types of thyroid cancer
Physical Examination of the Thyroid Gland
- Inspection: look for thyroid gland enlargement
- Palpation: feel for thyroid gland nodules or tenderness
- Auscultation: listen for thyroid gland sounds
- Percussion: tap on the thyroid gland
- Ultrasound: evaluate thyroid gland structure and function
