Hydatid Liver Disease

  1. Definition: A condition caused by the Echinococcus parasite, which leads to the formation of hydatid cysts in the liver.

  2. Types:

    1. Unilocular cyst: A single cyst that is usually large and filled with fluid.
    2. Polycystic disease: Multiple small cysts that can be scattered throughout the liver.
    3. Alveolar cyst: A rare type of cyst that is filled with a network of small cysts.
  3. Causes: The disease is caused by the Echinococcus parasite, which is usually transmitted through contact with contaminated feces or urine of infected animals.

  4. Clinical Features:

    1. Abdominal pain and tenderness: Pain in the right upper quadrant of the abdomen and tenderness on palpation.
    2. Jaundice: Yellowing of the skin and eyes due to bile duct obstruction.
    3. Itching: Itching sensation in the skin due to bile duct obstruction.
    4. Weight loss: Loss of appetite and weight due to chronic illness.
    5. Fatigue: Feeling of tiredness and weakness due to chronic illness.
  5. Investigations:

    1. Imaging studies: Ultrasound, CT scan, or MRI to visualize the cysts and their location.
    2. Blood tests: To check for evidence of the parasite and liver function.
    3. Serological tests: To detect antibodies against the parasite.
  6. Complications:

    1. Rupture of the cyst: Spontaneous rupture of the cyst, leading to peritonitis and other complications.
    2. Infection: Bacterial or fungal infection of the cyst.
    3. Obstruction of the bile duct: Blockage of the bile duct, leading to jaundice and other complications.
    4. Portal hypertension: Increased pressure in the portal vein, leading to varices and other complications.
  7. Management:

    1. Medical treatment: Anti-parasitic medication, such as albendazole, to kill the parasite.
    2. Surgical treatment: Drainage or removal of the cyst, or in some cases, liver resection.
    3. Supportive care: Management of symptoms, such as pain and jaundice, and monitoring for complications.
  8. Treatment:
