**TRAUMATIC HERNIA **
Definition
- Traumatic hernia is a type of abnormal protrusion of an organ or tissue through a weak area in the abdominal wall.
- It occurs due to a sudden increase in intra-abdominal pressure, usually as a result of a trauma.
Types
- Internal hernia - hernia occurs within the peritoneal cavity
- External hernia - hernia occurs outside the peritoneal cavity
- Incisional hernia - hernia occurs through a scar or incision in the abdominal wall
- Ventral hernia - hernia occurs in the midline of the abdominal wall
- Umbilical hernia - hernia occurs near the navel
- Spigelian hernia - hernia occurs through the Spigelian fascia, which is the aponeurotic layer between the rectus abdominis muscle and the semilunaris line
Causes
- Sudden increase in intra-abdominal pressure due to trauma
- Weakness in the abdominal wall
- Increased abdominal pressure due to coughing, straining, or heavy lifting
- Previous surgical incision or scar
Clinical Features
- A visible bulge or swelling in the abdominal wall
- Pain or discomfort in the affected area
- Abdominal tenderness or guarding
- Nausea or vomiting
- Abdominal distension
Investigations
- Physical examination
- Imaging studies such as X-rays, CT scans, or ultrasound
- Endoscopy or laparoscopy to visualize the hernia
Complications
- Intestinal obstruction or strangulation
- Internal herniation
- Adhesions or scar tissue formation
- Recurrence of the hernia
Management
- Conservative management - bed rest, pain management, and observation
- Surgical repair - hernioplasty or herniorrhaphy
- Laparoscopic repair - minimally invasive surgical technique
- Open repair - traditional surgical technique
Treatment
- Hernioplasty - mesh repair of the hernia
- Herniorrhaphy - suture repair of the hernia
- Laparoscopic hernia repair - minimally invasive surgical technique
- Open hernia repair - traditional surgical technique
