RENAL CARBUNCLE
Definition of Renal Carbuncle
- Renal carbuncle is an abscess in the kidney caused by the spread of infection from the renal pelvis or calyces, through the renal parenchyma.
- It presents as a localized area of renal inflammation and infection.
Types of Renal Carbuncle
- Pyogenic renal carbuncle: caused by bacterial infections such as Staphylococcus aureus.
- Tuberculous renal carbuncle: caused by Mycobacterium tuberculosis.
- Echinococcal renal carbuncle: caused by Echinococcus granulosus.
Causes of Renal Carbuncle
- Bacterial infections
- Tuberculosis
- Fungal infections
- Echinococcal disease
- Trauma or surgery to the kidney
Clinical Features of Renal Carbuncle
- Severe flank pain
- Fever
- Chills
- Vomiting
- Weight loss
- Abdominal tenderness
- Flank mass
- Urinary tract infection symptoms
Investigations for Renal Carbuncle
- Urinalysis
- Blood tests for infection or inflammation
- Imaging studies (CT or MRI) to visualize the kidney and abscess
- Biopsy to diagnose the cause of the abscess
Complications of Renal Carbuncle
- Perforation of the kidney
- Spontaneous rupture of the abscess
- Fistula formation
- Chronic kidney disease
- Sepsis
Management of Renal Carbuncle
- Antibiotics to treat the infection
- Drainage of the abscess through percutaneous or surgical methods
- Supportive care to manage symptoms and complications
Treatment of Renal Carbuncle
- Antibiotics: ciprofloxacin and ceftriaxone
- Drainage: percutaneous drainage or open surgery
- Supportive care: pain management, hydration, and nutrition
Blood Supply of the Kidney
- The blood supply to the kidney comes from the renal arteries, which branch from the abdominal aorta
- The renal arteries then divide into segmental and interlobar arteries
- The interlobar arteries supply the renal medulla and cortex
Nerve Supply of the Kidney
- The kidneys receive sympathetic and parasympathetic innervation
- The sympathetic nerves originate from the thoracic and lumbar spinal cord
- The parasympathetic nerves originate from the sacral spinal cord
Clinical Anatomy of the Kidney
- The kidneys are located in the retroperitoneal space
- The kidneys are surrounded by perirenal fat and the renal fascia
- The renal cortex is located outside the renal medulla
