Prostatitis

Definition

  • A type of inflammation of the prostate gland.
  • Generally associated with urinary symptoms.

Types of Prostatitis

  1. Acute Bacterial Prostatitis
    • A sudden onset of severe pain and inflammation of the prostate.
    • Often associated with infection.
  2. Chronic Bacterial Prostatitis
    • A long-standing inflammation of the prostate gland.
    • May be associated with recurring infections.
  3. Chronic Nonbacterial Prostatitis
    • A long-standing inflammation of the prostate gland not caused by bacteria.
    • Also known as chronic pelvic pain syndrome.
  4. Asymptomatic Inflammatory Prostatitis
    • Inflammation of the prostate gland without symptoms.

Aetiology of Acute Prostatitis

  1. Bacterial Infection
    • Most common cause.
    • Bacteria can enter the prostate through the urethra.
  2. Sexual Intercourse
    • Can increase the risk of infection.
  3. Urinary Tract Infections
    • Can spread to the prostate gland.
  4. Catheterization
    • Can introduce bacteria into the prostate gland.

Clinical Features of Acute Prostatitis

  1. Severe Pain
    • Pain in the lower abdomen, testicles, and penis.
    • Pain may radiate to the back and thighs.
  2. Fever and Chills
    • May be present with severe pain.
  3. Urgency and Frequency
    • Urinary urgency and frequency.
  4. Dysuria
    • Painful urination.

Clinical Features of Chronic Prostatitis

  1. Pain
    • Pain in the lower abdomen, testicles, and penis.
    • Pain may radiate to the back and thighs.
  2. Urinary Symptoms
    • Urinary urgency and frequency.
    • Dysuria or painful urination.
  3. Recurrent Infections
    • May experience recurring urinary tract infections.

Diagnostic Tests for Prostatitis and Their Interpretation

  1. Digital Rectal Examination (DRE)
    • May feel a tender or swollen prostate gland.
  2. Urinalysis
    • May show evidence of infection (e.g., leukocytes, bacteria).
  3. Prostate-Specific Antigen (PSA) Test
    • May be elevated in men with prostatitis.
  4. Urodynamic Studies
    • May show abnormalities in bladder function.
