Barium Enema

Definition

  • A barium enema is a medical imaging procedure used to visualize the large intestine (colon).
  • It involves the ingestion or infusion of a barium sulfate solution into the colon, which provides contrast for X-ray imaging.

Types of Barium Enema

  • Single-contrast barium enema: Uses barium sulfate only for contrast.
  • Double-contrast barium enema: Uses both barium sulfate and air or carbon dioxide for contrast.
  • Barium sulfate enema: Uses barium sulfate only for contrast.
  • Air-contrast barium enema: Uses air or carbon dioxide for contrast.

Indications

  • Suspicion of colon cancer
  • Colon polyps
  • Diverticulosis
  • Inflammatory bowel disease (IBD)
  • Intestinal obstruction
  • Intussusception
  • Meckel's diverticulum

Contraindications and Risks

  • Contraindications:
    • Severe constipation
    • Intestinal perforation
    • Intestinal obstruction
      • Bowel preparation not possible
  • Risks:
    • Allergic reactions to barium
    • Intestinal perforation
    • Colonic bleeding
    • Bowel obstruction

Patient Preparation

  • Cleanse the bowel with a bowel preparation solution
  • Avoid food and water for several hours before the procedure
  • Wear loose, comfortable clothing

Procedure

β†’ Bowel preparation β†’ Insertion of rectal tube β†’ Infusion of barium sulfate solution β†’ X-ray imaging β†’ Removal of rectal tube β†’ Monitoring of patient

Normal Findings

  • Normal colon diameter and shape
  • No polyps or tumors
  • No signs of inflammation or infection

Abnormal Findings

  • Colon polyps or tumors
  • Diverticulosis
  • Inflammatory bowel disease (IBD)
  • Intestinal obstruction
  • Intussusception
  • Meckel's diverticulum

Blood Supply

  • Colon: Superior mesenteric artery (SMA)
  • Rectum: Inferior rectal branches of the internal iliac artery

Nerve Supply

  • Colon: Anterior and posterior mesenteric plexuses
  • Rectum: Inferior hypogastric plexus

Clinical Anatomy

  • Colon: A long, tubular structure with 4 parts (ascending, transverse, descending, sigmoid)
  • Rectum: A short, narrow structure at the end of the colon
